Form Completion Instructions:
Registration requires completion of one form, the .

- Complete the player registration form in its entirety.

- For Fall Registrations: All registrations must be submitted by June 30th.
- For Spring Registrations: All registrations must be submitted by January 31st.
- All registrations submitted after these deadlines must pay the late fee of $25 per family.
- No registrations will be accepted 15 days after the registration deadlines.
- All in-house players (Ages 5 through 10) must have a Uniform (consisting of a reversible blue/gold shirt and blue socks) prior to the first game. Uniforms can be ordered from: .

- All travel players (Ages 9 through 19) must have a Uniform (consisting of a shirt, shorts and socks) prior to the first game. Uniforms can be ordered from: .

Additional notes:
- If your child is a new player you must furnish a copy of their Birth Certificate.
- If your child is a member of a travel team, you must furnish a 1"x 1" photograph and the player must sign a player card.
- Completed forms can be dropped off in person at the MASC fields during registration (to be announced) signups.
- Uniforms can be ordered with the .
- For New Players: Follow the steps on the Order Form.
- For Returning Players: It is not necessary to purchase a uniform every year. You only need to purchase a new uniform (either the shirt, socks or both) if it has torn, or the player has outgrown it. Replacements can be purchased through the MLS Soccer store.
- Uniforms for ages U10 Travel through Age 19 can also be purchased directly from and are not included with the registration fees.
